Insurance & Payment Information

  • We are not an insurance based clinic.

  • Insurance clinics sell access. We sell best outcomes, time with our patients and expertise.

  • We are out-of-network for all insurance plans.

  • We provide a superbill so you can submit for reimbursement. Most plans reimburse up to 85%.

  • Labs can be billed to your insurance directly.

  • Medicare, Medicaid, and AHCCCS do not reimburse for naturopathic services or labs.

  • Cash, credit card, HSA and FSA, Afterpay, and Klarna payments are accepted.

Our Insurance Approach

Chapter One Wellness is an out-of-network provider. This means we do not bill insurance directly for our office visits or services.  Instead, we focus on:

  • Longer appointment times; typically 90 minutes for new patients

  • Individualized treatment plans for better health outcomes

  • Expanded holistic options

  • Dedication to finding root cause of illness

​

After your visit, we provide a superbill (an itemized receipt with diagnosis and procedure codes) that you can submit to your insurance company for reimbursement.

What’s Covered — and What’s Not

Medical Visits

  • Out-of-network

  • Pay at time of service

Lab Services (LabCorp, Sonora, etc.)

  • Labs are covered by commercial insurance

  • Labs are drawn at our office and sent to the lab

  • The lab bills your insurance directly, you will not be charged out of pocket for labs​

Aesthetics (Botox, fillers, PRP, IVs,  skin tightening, weight-loss injections, hormone pellets, etc.)

  • Most insurance plans do not cover these services

  • Cash pay only

Medicare / Medicaid / AHCCCS

  • Not accepted

  • No reimbursement available

Note: Check your plan’s out-of-network benefits. Submission of a superbill does not guarantee 100% reimbursement. Most insurances will reimburse approximately 85%.

How to Use a Superbill

  1. Pay for your visit at the time of service.

  2. We give you a superbill with all necessary codes.

  3. Submit it to your insurance company (mail, portal, or app).

  4. Your insurance processes your claim and reimburses you directly.

Tips for Checking Your Benefits

Before your appointment, call the member services number on your insurance card and ask:

  • “Do I have out-of-network benefits for office visits with a naturopathic physician?”

  • “What is my deductible and coinsurance for out-of-network services?”

  • “Are labs sent through LabCorp or Sonora Quest covered under my plan?”

Insurance FAQs

 

What does “out-of-network” mean?

It means Chapter One Wellness is not contracted with any insurance company. You pay for your visit at the time of service, and we give you a superbill to submit to your insurer. Depending on your plan’s benefits, you may receive reimbursement directly from your insurance company.

What is a superbill?

A superbill is a detailed receipt with all the information your insurance company needs: diagnosis codes, procedure codes, provider information, and your payment details. You send it to your insurance company, and they process it for possible reimbursement.

Does Medicare, Medicaid, or AHCCCS cover visits here?

No. Unfortunately, these programs do not reimburse for our services or labs. Payment is due at the time of service. Comprehensive lab testing costs $225 out of pocket or we can work with your Medicare/Medicaid/AHCCCS provider to order labs through a conventional lab company

Are labs covered by insurance?

Yes — labs collected at our office are sent to LabCorp or Sonora Quest which bills your insurance directly. This means you typically pay only what your insurance requires (deductible, copay, coinsurance).

Do you accept HSA or FSA cards?

Yes. We accept Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) as well as cash and major credit cards.

How do I find out if I’ll be reimbursed?
